Testicular Torsion

Testicular torsion is a medical emergency that occurs when a testicle rotates, twisting the spermatic cord that provides blood flow to the testicle. This twisting can block blood flow to the testicle, leading to ischemia (lack of blood supply) and potentially causing permanent damage to the testicular tissue. Testicular torsion is a painful condition that requires immediate medical attention to prevent complications.

Causes

  • Testicular torsion can occur spontaneously or be triggered by physical activity, trauma, or strenuous exercise.
  • In some cases, there might be an anatomical predisposition that allows the testicle to rotate more easily, increasing the risk of torsion.

Symptoms

  • The most common symptom is sudden, severe testicular pain that is often accompanied by swelling and redness of the scrotum.
  • The pain might radiate to the lower abdomen or groin area.
  • Nausea and vomiting can also occur due to the intense pain.

Diagnosis

  • Testicular torsion is diagnosed based on the characteristic symptoms and a physical examination.
  • A healthcare provider might perform a manual detorsion (attempt to untwist the testicle) to see if the pain is relieved. However, if pain persists, the condition is considered a surgical emergency.

Treatment

  • Testicular torsion requires immediate surgical intervention to restore blood flow to the affected testicle. If not treated promptly, it can lead to permanent damage and loss of the testicle.
  • During surgery (orchiopexy), the surgeon untwists the testicle and fixes it in place to prevent future torsion.

Prognosis

  • The prognosis for testicular torsion depends on how quickly treatment is provided. If surgery is performed within a few hours of the onset of symptoms, the testicle can often be saved.
  • However, delayed treatment can lead to irreversible damage and the need for the affected testicle to be removed.

Prevention

  • Testicular torsion cannot always be prevented, especially if it occurs spontaneously. However, men with a history of testicular pain or who have an anatomical predisposition may be at higher risk.
  • Being aware of the symptoms and seeking prompt medical attention at the first sign of severe testicular pain can help prevent complications.
